WHO QUALIFIES

Does Your Loved One Qualify?

To be eligible for the GUIDE Program, your loved one must meet all three criteria below.

Has dementia — we can help you obtain a diagnosis if appropriate

✓

Enrolled in Traditional Medicare Parts A & B — No Medicare Advantage Plans

✓

NOT currently on hospice or in a skilled nursing facility — short-term stays are OK

Also eligible: VA Patients, Dually Eligible Medicaid Patients, Memory Care residents, and Palliative Care patients may qualify. Call us to confirm — we'll check for you.

Who Qualifies for the 72 Free Hours of Home Care?

To receive the annual 72 hours of no-cost in-home care through BlueBonnet, individuals must have moderate to severe dementia and have a family or unpaid caregiver — the caregiver does not need to live with the person with dementia. Annual hours may vary slightly each year per Medicare guidelines.
